The terminals seem to be numbered across the top from left to right 1 thru 6 and across the bottomfrom right to left from 7 thru 11 but there are other terminals marked 5, 6, 7, and 8 that are not at the edge. See the next picture and you will see the other 5 near the bottom of the fuse.
